How does the Americans with Disabilities Act (ADA) define a disability ?

The ADA considers a condition to be a disability only if the following three criteria are met:

• The condition must be a physical or mental impairment that substantially limits one or more major life activities
  
  
• The impairment must be documented
  
  
• The individual must be regarded as having such an impairment
